NOTE: You can fill the application out on your computer, but you cannot save it. After you have filled it out, print it, and follow the directions on page 2 to submit your application. After you close the application window, your information will NOT be saved.

Failure to up date the board will cause your contact listing on the web site to be inaccurate and may cause your renewal information to not reach you in time for renewal. This could lead to additional reinstatement fees.
